Call 214.321.0156, ext. 3118 to set up an appointment for an assessment. This screening phone call will only take about 10 minutes. During the phone call, an admissions counselor will set up an appointment for an assessment. The assessment takes about an hour and contains questions about medical history, drug and alcohol use history, living arrangements, family support and economic situation. During the assessment, the admissions counselor will obtain information about insurance, NorthSTAR eligibility and determine the need for funding from other sources as needed.

Required Documents
Clients coming into Nexus for admission will need the following paperwork:
- Picture ID (if available)
- Proof of residency is required by NorthSTAR and DSHS. Anything like a bill, driver's
license, check etc. that has your address printed on it shows proof of residency. - Social security card (if available)
- If you are employed, proof of income is required by NorthSTAR and DSHS.
A parent or guardian must be present when an adolescent client is admitting. The parent or guardian must bring shot records and withdrawal paperwork from the adolescent's current school. They must also bring a birth certificate for the adolescent, if she is not already enrolled in DISD. Parents, please also bring your proof of income so we can work with you to determine the best possible payment options.
There are additional document requirements for children accompanying their mothers into treatment:
- Social security card (if available)
- A withdrawal form from the child's present school is required.
- Birth certificate (if available)
- Shot records are required.
