Make An Appointment

The first step is to call admissions at (214) 321-0156 x3118. This screening phone call will take about 10 minutes. During the phone call, an admissions counselor will set up an appointment for an assessment.

The assessment contains questions about medical history, drug and alcohol use, living arrangements, family support and economic situation. During the assessment, the admissions counselor will obtain information on insurance and/or eligibility for NorthSTAR and other sources as needed. Adolescent clients must be accompanied by a parent or guardian during the admissions process. The average time in admissions is approximately three hours. 



What to bring to your assessment:
All Clients
• Picture ID (if available)
• Proof of residency is required by NorthSTAR and DSHS. Examples include a bill, driver's
license, or check etc. that displays your printed address
• Social Security card (if available)
• If you are employed, proof of income is required by NorthSTAR and DSHS
Residential Clients
Bring one bag only of clothing/personal items to your scheduled admissions appointment. Don't forget your prescription medications! Please do not bring cell phones, computers, outside food, mouthwash or other products containing alcohol, linens, or items of excessive value.
Adolescents
A parent or guardian must be present when an adolescent client is admitting and bring the following:
• Immunization records
• Withdrawal paperwork from the adolescent's current school
• Birth certificate for the adolescent, if she is not already enrolled in DISD
• Proof of income so we can work with you to determine the best possible payment options
Mothers Bringing Children
There are additional document requirements for children accompanying their mothers into treatment:
• A withdrawal form from the child's present school is required
• Immunization records are required
• Social Security card (if available)
• Birth certificate (if available)
